Summary Report an accident when someone has been hurt or property has been damaged. From the PaT Menu you open an accident form, give it a title, choose where the accident happened (adding a new location if it isn't listed) and describe what happened as best you can. Submitting the form creates a task for a qualified person to investigate the accident, and brings it to the attention of the Health and Safety committee.
Steps
Step 1
Open the PaT Menu: click the round + button at the bottom centre of the screen. A fan of round icons opens above it.
Step 2
Click the red icon with a white warning triangle, at the top of the fan. Its label reads Add a new Accident report. A message box opens in the middle of the screen.
Step 3
The box asks you to confirm that you want the accident form, which is only for when someone has been hurt or property has been damaged. If so, click Accident at the bottom of the box. The other button, Near Hit - Hazard, lodges a near hit on the Hazard form instead; the Capture and Manage Hazards module covers hazards.
Step 4
The Create an Accident Form box opens. Type a title for the accident in Enter the title of this Accident, then click Open an Accident Form below it. The accident form opens, with your title at the top.
Step 5
Fill out the location first, at the top of the form: choose the branch or site where the accident happened in Branch/Site?, then the area within it in Location?.
Step 6
If the location is not listed in the Location? dropdown list, add a new location: click the + icon at the right of Location?. A box opens for managing your location choices.
Step 7
Type the new location in Choice Value, then click Submit at the bottom right of the box.
Step 8
Fill out the rest of the form, row by row: When (the date and time of the accident), Where (more detail about the location), What (such as the injury type, and whether treatment was sought), How (the details of the accident, the people involved and any witnesses) and Who was injured. Check that the date and time are accurate, because PaT uses them in calculations. Just do the best you can: this is not a test, and there is no right or wrong. The important thing is to record what happened to the best of your ability.
Step 9
When you have finished, click Submit to Supervisor at the bottom of the form, to the right of Print Page and Close.
Step 10
Submitting the form triggers two important actions: - A task is created automatically, assigning a qualified person to conduct a full investigation of the accident. - A copy of this task is added to the company's Health and Safety committee meeting minutes. So by notifying your employer of an accident, you're not just triggering a formal investigation: you're also bringing the issue to the attention of the Health and Safety committee. The committee then reviews the investigator's findings to confirm that the company has taken all reasonably practicable steps to protect you and your colleagues from harm, and to prevent a similar incident from happening again.
