Summary Report a hazard or near miss. From the PaT Menu you open a hazard form, give the location, describe the hazard, choose its category, rate its likelihood and severity, and suggest what should be done. Submitting it creates a task for your supervisor or Health and Safety officer, and if it isn't dealt with within 5 days, it escalates.
Steps
Step 1
Open the PaT Menu: click the round + button at the bottom centre of the screen. A fan of round icons opens above it.
Step 2
Click the yellow icon with a white warning triangle, at the top of the fan, just right of the red one. Its label reads Add a new Hazard/Near miss Report. The Create a new Hazard/Near miss Form box opens in the middle of the screen.
Step 3
Give your report a short title: type it in the box under Enter a brief Title for this Hazard/Near Miss.
Step 4
Click Open a Hazard Form, below the title box. The hazard form opens, with your title at the top.
Filling in the hazard form
Step 5
Fill in the location first, in the Location of Hazard or Near Hit row at the top of the form: choose the branch from the first drop-down list.
Step 6
Then choose the area at that branch from the second drop-down list, just to its right.
Step 7
Describe the hazard or near hit in the What is the Hazard or Near Hit? box, on the left, below the location and date.
Step 8
Select the type of hazard from the Hazard Category? list under Select Category, to the right of your description.
Step 9
Rate the hazard, using the two tables under Rate the Hazard: - Risk: use your judgement to rate the likelihood of someone being injured if they were exposed to the hazard. Select the number that best reflects this, from 1 (Rare) to 5 (Almost Certain). - Severity: use your judgement to assess how serious the harm or injury could be if an incident occurred, from 1 (Insignificant) to 5 (Catastrophic). Once both are rated, multiply the two numbers using the matrix provided, below the tables, to calculate the overall hazard rating.
Step 10
Enter the hazard rating in the drop-down list beside The Rating that I give this hazard is (Risk x Severity = Hazard Rating), below the matrix. For example, a Risk of 1 and a Severity of 2 give a hazard rating of 2 (1 x 2 = 2). The Risk table beside the matrix shows what each band of ratings calls for, from Low (1-5) to Extreme (15-25).
Step 11
In What do you think should be done about it?, at the bottom of the form, let your supervisor know what actions you believe should be taken to manage this hazard and ensure everyone's safety.
Step 12
Click Submit to Supervisor, at the bottom of the form, to the right of Print Page and Close.
What happens next
Step 13
Submitting the form automatically creates a task in PaT and sends it to either your supervisor or the Health and Safety officer assigned to your user profile. They have 5 days to address the issue, and must report back to you once it has been resolved. You are then asked to confirm whether you are satisfied with their response.
Step 14
If the issue is not addressed within 5 days, the assigned person's Task Integrity KPI is reduced, and this stays on their record for 3 months. At the same time, a new task is automatically created for their supervisor, who then has 5 days to resolve the hazard. If they also fail to act, the task escalates again, continuing up the management chain until it reaches the CEO (or the most senior manager), who may be legally liable for failing to manage hazards appropriately, including the risk of fines or jail time. This auto-escalation process ensures accountability at every level.
Step 15
Raising hazards in PaT is a powerful way to protect both yourself and your colleagues from potential harm. It also helps protect senior managers, who could face significant fines or even jail time if hazards are not properly managed. By reporting issues, you play a key role in keeping everyone safe and the organisation compliant.
